This patient was receiving dialysis as an outpatient and was admitted with decreased LOC and hypotension. He was clearly fluid overloaded, however was unable to tolerate fluid removal or even ultrafiltration due to profound hypotension when connected to the dialysis machine. Continuous veno-venous hemodialysis (CVVHD) was also tried unsuccessfully. At the time he was transferred to the CCU, he had not had dialysis for nearly a week. What diagnosis can you make from this ECG strip?
